Waiver/ Indemnification

Parent or legal guardian must complete this form and make the payment before the child is accepted into the program.

Cancelation: I understand that all enrollments are final and No refunds will be issued once payment has been made.

Agreement: I agree to indemnify and hold harmless Tennis 4 FUN  and its coaches from any and all liability, damage, cost or expense arising my child’s participation in the program, of every kind  and nature.

In the event that I cannot be reached during an emergency, I hereby authorize Tennis 4 FUN to administer or arrange for any medical treatment deemed necessary for my child.

I also give Tennis 4 FUN permission to communicate with me via text and email and to take pictures and/or videos of my child for advertising and marketing purpose.
